Unifying action notice emails

Currently all action notice emails do their headers in a uniform way, with the following exceptions:
1. From is different. For comments, from is always the person who created the comment (with exceptions discussed in http://deme.stanford.edu/viewing/textcomment/83 ). But in every other action notice, from is "Deme Notifier" (with the email address where you may reply to the relevant item).
2. Subject is different (the only difference is instead of the comment name, it says "Todd created Deme Email integration plan", etc.
Let's decide how to treat the from field, and from there see what makes the most sense for the subject/body of these other action notices. Let me know what you think.
